Adding Pages to Your U.S Passport

Delphi can assist you with adding pages to your passport if:

• Your passport is valid for at least 1 year; AND
  Your passport is in good condition; AND
  You need additional visa pages to travel; AND
  Your name has not changed since your current valid passport was issued

You Will Need the Following Documents:

1. DS-4085 Application for Additional Visa Pages

• At this link Click Here you are required to fill in the form online where a specifically generated bar code is produced for you. Forms not filled in online will not be accepted US Department of State.

2. Two Original Letters of Authorization
Complete and sign 2 identical Letters of Authorization. 2 print automatically. [Once you have talked to one of Delphi’s agents, they will email you template letters for you].
One copy will go inside the sealed envelope and one should be attached to the outside of the envelope. This will be explained to you in further detail once you have contacted us.

3. Proof of Departure
Please provide ONE of the following

• A photo-copy of your airline ticket or a copy of your itinerary from your travel agent, airline, or online booking company
• A letter detailing your business travel. Delphi can provide you with a sample letter to follow in order to expedite the process.
• A letter explaining that you will be driving to Canada and/or Mexico.  Include intended departure date, intended return date, and destination.

4. Payment
There are 3 fees associated with this service that can be paid with all major credit cards or a money order
